Rivervale Nursing Home is registered for up to 43 residents and can support adults with low to maximum dependency needs. Each person’s needs are assessed before admission where possible, so the team can confirm whether the home can provide the right level of care.
Care provided at Rivervale includes general long-term care, respite care, convalescent care, palliative care, and care for residents living with dementia or memory-related conditions.
Care levels and nursing cover
Rivervale provides 24-hour nursing care. The nursing and care team supports residents with assessed clinical, personal, and dependency needs, including complex illness, reduced mobility, frailty, dementia, palliative needs, and changing health status.
Staffing levels are reviewed against residents’ dependency levels and care needs. The team includes registered nurses, healthcare assistants, administrative staff, and ancillary staff, with allied-health professionals involved where required.
Assessment and care planning
A nursing assessment is completed before admission where possible. Admission is confirmed only where the centre can meet the needs identified in that assessment.
Each resident has a care plan based on assessed needs. Care plans are reviewed regularly and updated where there is a change in condition, dependency, treatment, medication, or support requirement. Residents and families are involved in reviews where appropriate.
The resident’s GP is involved in planned and emergency admissions where possible. Medical information is gathered as soon as possible after admission so that the care plan reflects current clinical needs.
Clinical and allied-health support
Care at Rivervale is delivered with input from the resident’s medical practitioner and relevant allied-health services. Depending on assessed need, this may include physiotherapy, occupational therapy, pharmacy, speech and language therapy, dietetics, tissue viability nursing, phlebotomy, sub-cutaneous fluids, and support with hospital appointments.
Care-plan reviews include changes in condition, treatment, and medication. Palliative care is provided in collaboration with the resident’s GP and local palliative care services where required, with a focus on clinical comfort, symptom management, and clear communication with the resident and family.
Residents have access to advocacy support where needed.
